AI Failures Are Inevitable. So Is the CIO Getting Blamed.
CIO, Thursday, September 17th, 2026
52% of IT leaders say CIOs take the blame for agent mistakes; just 6% point to legal or compliance.
A survey commissioned by 8x8 found 52% of IT leaders say CIOs are blamed when an AI agent goes wrong, against just 6% who say legal or compliance leaders shoulder responsibility.
The problem is that CIOs often did not choose the vendor and do not have the audit trail: tools were approved by boards, other executives or business units, adopted as shadow IT, or shipped by vendors directly into platforms running mission-critical applications, creating a governance vacuum.
8x8's Morgan Watts traces it to the rush to adopt a year and a half ago without governance frameworks or visibility controls, and argues CIOs must now become AI governance evangelists because the job cannot be delegated.
